Change of Circumstances: Outline
A Change of Circumstances form must be completed if there have been any changes to your circumstances since completing your original housing application form. You must contact us at Dale & Valley House so we can make the changes for you.
These circumstances could include change of address; household members; medical / welfare needs or reasons that you require re-housing. Failure to provide change of circumstances information could result in an offer being withdrawn.
